How To Register

To register with Brewood Medical Practice please complete a Patient Registration Form. This can be collected from reception or downloaded using the link below.

Print off a registration form (GMS1) for each person wishing to register along with an adult health questionnaire for people aged over 16 years. For anyone under the age of 16, please complete the child under 16 years questionnaire.

Completed forms need to be handed into the surgery and your registration process completed before we can offer any appointments/services at the surgery.

Providing identification at the point of registration can help us trace your medical records easily, but this is not essential.

After registration:

An appointment will be made for you to see the nurse or health care assistant, for a new patient medical. If you are taking any medication, you will need to see a GP. You have the choice of seeing any of the GP’s listed.

Patients are within their rights to request sight of their medical records. A written request is initially required. 

Please note that this practice does not tolerate any kind of verbal or physical abuse towards members of the practice team or other patients. Doctors, therefore, reserve the right to remove patients from their list.

Important!

If your contact details change including your address, telephone number, email address or chosen method of contact please let us know either by telephoning the surgery or emailing brewood.surgery@nhs.net.

If your contact details are to be used for another patient, for example a family member or friend, you will need to complete a third-party authorisation form. This is to ensure the patient gives full consent for you to be able to speak on their behalf.
